Healthcare At Home AFC LLC is proud to offer comprehensive Adult Family Care (AFC) services, also known as Adult Foster Care, designed to provide compassionate, personalized care for individuals who need assistance with daily living activities. Our program ensures that both the individual requiring care and their caregiver—whether a family member or not—receive the support they need for a safe, healthy, and fulfilling life.

Eligibility Requirements for AFC Membership

To join our Adult Family Care program, individuals must meet the following criteria:

  • Be 16 years of age or older
  • Be eligible for Medicaid (MassHealth)
  • Require 24-hour live-in assistance
  • Need help with one or more Activities of Daily Living (ADLs), such as:
    • Bathing
    • Dressing
    • Walking
    • Transferring (e.g., getting in and out of bed)
    • Using the bathroom
    • Eating

Comprehensive Care and Support for Caregivers

We understand that being a caregiver is a demanding and vital responsibility. That’s why we offer a range of services to ensure the highest standards of care and support:

  • Initial Safety Assessment: We assess the care setting to ensure compliance with all MassHealth safety requirements.
  • Thorough Screening: Each caregiver undergoes a comprehensive screening process, including reference checks, criminal history evaluations, and up-to-date medical information.
  • Training and Ongoing Support: Our caregivers receive professional training, guidance, and continual support to deliver the best care possible.
